Overview

An ulcer is a discontinuity or break in an epithelial surface, such as the skin or a mucous membrane, that extends through the epithelium into underlying tissue and fails to heal in the normal timeframe. Ulcers arise wherever tissue injury outpaces repair, and their causes include impaired perfusion, pressure, infection, chemical or drug exposure, immune-mediated inflammation, venous and arterial insufficiency, and malignancy. They are classified by site and mechanism, encompassing peptic and oral Ulcers of the gastrointestinal tract, diabetic foot and lower-limb Ulcers, corneal Ulcers, pressure injuries, and ulceration secondary to medication or neoplasia. Pathophysiology centers on the disruption of the mucosal or cutaneous barrier, persistent inflammation, microbial colonization, and, in chronic wounds, a stalled healing trajectory associated with neuropathy or ischemia. Management combines treatment of the underlying cause with measures to control infection, optimize perfusion, and promote epithelialization.
