Overview

Basal cell carcinoma is a malignant tumour arising from the basal keratinocytes of the epidermis and its appendages, and is the most common form of non-melanoma skin cancer. It is driven largely by cumulative ultraviolet exposure and by dysregulation of the Hedgehog signalling pathway, and it characteristically arises on sun-exposed sites such as the face, head, and neck. Clinically it presents in several subtypes, including nodular lesions that appear as pearly, telangiectatic papules, superficial, morphoeic or infiltrative, and pigmented variants, each with differing growth behaviour. Basal cell carcinoma is typically slow-growing and only rarely metastasises, but if neglected it invades and destroys adjacent tissue, with disfigurement a particular risk on the face. Diagnosis rests on clinical and dermoscopic assessment confirmed by histopathology, and treatment is selected by subtype, size, and location, encompassing surgical excision, Mohs micrographic surgery, curettage, cryotherapy, topical agents such as imiquimod, ablative laser approaches, and targeted Hedgehog-pathway inhibition for advanced disease. The literature collected here addresses combined laser microporation with topical imiquimod for actinic keratosis and basal cell carcinoma, reconstructive flap repair of facial defects, and the differential diagnosis of other cutaneous and adnexal tumours, situating basal cell carcinoma within the wider study of skin cancer and dermatologic oncology.
