Overview
Vasculitis is a group of disorders characterised by inflammation of blood vessel walls, which can damage the vessels, restrict or block blood flow, and lead to ischaemia and injury of the tissues and organs they supply. The conditions are classified principally by the size of the vessels affected, large, medium, or small, and by whether the inflammation is primary, arising as an idiopathic or autoimmune process, or secondary to infection, connective tissue disease, drugs, or malignancy. Clinical manifestations are highly variable and may include constitutional symptoms such as fever, fatigue, and arthralgia alongside organ-specific features reflecting the vascular beds involved, from the central nervous system in cerebral vasculitis to the heart, skin, kidneys, and other sites. Vasculitis can occur as part of recognised syndromes such as Behçet's disease and monogenic conditions including deficiency of adenosine deaminase type 2, which may mimic lupus, and it has also been described in association with infections and immunisation. Diagnosis relies on integrating clinical, laboratory, imaging, and histopathological findings to localise and characterise the inflammatory process, while treatment commonly involves immunosuppressive and anti-inflammatory therapy directed at controlling vascular inflammation and preventing organ damage. Understanding the mechanisms, classification, and presentations of vasculitis is essential for accurate diagnosis and for tailoring management to the type and severity of vascular involvement.
