Therapy Management Medication Therapy Management (MTM) is a form of health care program designed to optimize the use of medications and improve health outcomes for individuals. The aim of MTM is to optimize medication use for the purpose of improving medication adherence, preventing medication errors and reducing unnecessary costs. MTM involves conducting a comprehensive medication review, providing patient-specific education, and developing an individualized medication plan. Through MTM, pharmacists and other health care providers collaborate to ensure that the patient’s medications are effective, and are used in the most beneficial and cost-effective way. By providing education and reviewing medication efficacy and safety, MTM has the potential to improve health outcomes and reduce health care costs. In addition, it may improve medication adherence and reduce the risk of adverse drug events.
