Overview

Chronic sinusitis, more precisely termed chronic rhinosinusitis, is persistent inflammation of the mucosa lining the nasal cavity and paranasal sinuses lasting twelve weeks or longer, defined by symptoms such as nasal obstruction, mucopurulent discharge, facial pressure or pain, and reduced sense of smell. Its pathophysiology involves an interplay of impaired mucociliary clearance, obstruction of the sinus drainage pathways, mucosal oedema, and host responses to microbial colonisation, with the maxillary and ethmoid sinuses frequently involved. The condition is commonly classified by the presence or absence of nasal polyps, a distinction that informs both prognosis and management. Contributing factors include anatomical narrowing of the sinus ostia, allergic and inflammatory mechanisms, biofilm formation, and microbial agents implicated in mucosal disease and polyp formation. Assessment combines symptom-based instruments, nasal endoscopy, and cross-sectional imaging that characterises ostial dimensions and sinus anatomy relevant to ventilation and surgery. Management ranges from medical therapy directed at inflammation and infection to functional endoscopic sinus surgery that restores drainage, with reconstructive grafting used in selected procedures. Rarely, untreated sinus infection extends beyond the sinuses to cause serious complications such as venous sinus thrombosis, underscoring the importance of timely diagnosis and treatment.
