Overview

Cardiovascular disorders are diseases of the heart and blood vessels, encompassing conditions such as coronary artery disease and myocardial ischaemia, heart failure, hypertension, valvular and rheumatic heart disease, congenital cardiac defects, and arrhythmias. Within this broad field, pulmonary vascular diseases form a distinct subgroup affecting the circulation of the lungs, where deoxygenated blood is delivered from the right heart to the pulmonary capillaries for gas exchange and returned to the left heart. Pulmonary hypertension, the cardinal pulmonary vascular disorder, denotes abnormally elevated pressure in the pulmonary arteries arising from diverse mechanisms, including primary disease of the small pulmonary vessels, left-heart disease, chronic lung disease and hypoxia, and chronic thromboembolic obstruction. Sustained elevation of pulmonary pressure increases the workload of the right ventricle, which may hypertrophy and ultimately fail, linking pulmonary vascular pathology to systemic cardiovascular consequences. Other pulmonary vascular conditions include pulmonary embolism and vascular involvement secondary to congenital and acquired heart disease. The interdependence of the systemic and pulmonary circulations means that cardiac disorders such as heart failure and valvular disease frequently affect, and are affected by, the pulmonary vasculature. Diagnosis combines clinical assessment, imaging, and haemodynamic measurement, with biomarkers and emerging molecular tools aiding prognosis. Research addresses the mechanisms of pulmonary vascular remodelling, right-heart adaptation, and the diagnosis, risk stratification, and treatment of cardiovascular and pulmonary vascular disease.
