Overview

Hematological diseases are disorders that affect the blood and the blood-forming organs, encompassing both non-malignant and malignant conditions across every cellular and biochemical component of the hematopoietic system. They are organized by the affected element: disorders of red cells include the anemias and the polycythemias, such as polycythemia vera, a myeloproliferative neoplasm whose management with agents like hydroxyurea must be balanced against drug toxicity; disorders of white cells affect immune competence and are reflected in altered leukocyte and lymphocyte dynamics; and disorders of platelets and the coagulation system impair hemostasis. Abnormalities of hemoglobin structure, exemplified by sickle cell disease, produce chronic hemolysis and painful vaso-occlusive crises. The malignant disorders of blood and marrow, including the leukemias and lymphomas such as Hodgkin lymphoma, arise through leukemogenesis and clonal proliferation. The underlying pathophysiology generally reflects impaired production, accelerated destruction, intrinsic cellular dysfunction, or neoplastic proliferation of blood cells. Laboratory evaluation centers on the complete blood count, a foundational test that quantifies the cellular elements and guides further investigation, and on additional markers such as lactate dehydrogenase. Clinically, these diseases manifest through anemia, immunodeficiency that heightens vulnerability to infections including invasive fungal disease such as aspergillosis in immunocompromised patients, and bleeding tendencies, requiring careful diagnostic and supportive management.
