Overview

HIV/AIDS and substance abuse are interrelated public health concerns that frequently co-occur and influence one another. Substance use, including the use of injection drugs and the consumption of alcohol and other substances, can increase vulnerability to HIV infection through behaviors such as sharing of contaminated needles and engagement in unprotected sex, as well as through effects on judgment and the immune system. Once a person is living with HIV, substance use can complicate care by reducing adherence to antiretroviral therapy, interacting with medications, accelerating disease progression, and contributing to mental health challenges. Conversely, the stresses associated with HIV diagnosis and stigma may contribute to substance use. Addressing this intersection requires integrated approaches that combine HIV prevention, testing, and treatment with substance use screening, harm reduction, and behavioral and social support. Prevention efforts emphasize education, access to clean injection equipment where appropriate, treatment for substance use disorders, and the reduction of stigma that can deter individuals from seeking care.
