Overview

Vomiting, or emesis, is the forceful, coordinated expulsion of gastric contents through the mouth, produced by a reflex integrated in the brainstem emetic centre and triggered by gastrointestinal, vestibular, central, metabolic, or toxic stimuli. It serves a protective function against ingested toxins but is also a clinical sign of diverse disorders, and protracted vomiting can cause dehydration and electrolyte disturbance through loss of fluid and gastric secretions. Evaluation focuses on identifying the underlying cause and associated gastrointestinal or systemic pathology. Research relevant to this area examines acute diarrhoea and rotavirus infection in young children before and after vaccine introduction, gastric dilatation-volvulus as a cause of acute abdominal emergency, and granulomatous gastritis in a clinicopathologic biopsy series. Allied work addresses superior mesenteric artery syndrome presenting with upper-gastrointestinal symptoms, intragastric balloon treatment and its tolerability, intestinal protozoan infection, the management of severe acute malnutrition with therapeutic feeds, and adverse drug reactions involving gastrointestinal symptoms. These studies integrate gastroenterology, infectious disease, and clinical pathology to clarify the causes, presentations, and consequences of vomiting and related gastrointestinal disturbance.
