Overview

Rheumatoid arthritis is a chronic, systemic autoimmune disease in which the immune system attacks the synovial lining of joints, producing persistent inflammation, pain, swelling, and progressive destruction of cartilage and bone. It characteristically affects small joints of the hands and feet in a symmetric pattern and, untreated, leads to joint deformity, functional loss, and reduced quality of life. The disease is driven by autoreactive immune responses involving inflammatory cytokines, autoantibodies such as rheumatoid factor and anti-citrullinated protein antibodies, and proliferation of synovial tissue into an invasive pannus; cellular changes including increased synovial and cartilage cell populations contribute to joint damage. Because inflammation is systemic, extra-articular manifestations can involve the cardiovascular system, lungs, and eyes, and shared autoimmune mechanisms link it to related conditions including juvenile idiopathic arthritis. Diagnosis combines clinical assessment, serologic markers, inflammatory indices, and imaging, while genetic susceptibility and signaling pathways are studied to refine risk and treatment. Management has shifted toward early, targeted control using disease-modifying antirheumatic drugs, including biologic and small-molecule agents, alongside glucocorticoids for inflammation, with the goal of remission and preserved function. Emerging precision-medicine approaches aim to match patients to therapies based on molecular and immunologic profiles.
