Overview

Inflammatory arthritis denotes a group of joint disorders in which immune-mediated inflammation of the synovium and related structures produces pain, swelling, stiffness, and, over time, damage to cartilage and bone. Unlike degenerative joint disease, it is driven by dysregulated innate and adaptive immunity, with activated immune cells and pro-inflammatory cytokines sustaining synovial inflammation; many forms are autoimmune, in which tolerance to self-antigens is lost. The category includes rheumatoid arthritis, the juvenile idiopathic arthritides, and arthritis associated with systemic autoimmune and connective-tissue disease, as well as crystal- and infection-related joint inflammation that share inflammatory features. Characteristic manifestations include symmetrical or oligoarticular joint involvement, morning stiffness, and systemic features, and the inflammatory process is reflected in cytokine activity and serological and genetic markers of disease susceptibility. Diagnosis combines clinical assessment, laboratory and imaging studies, and exclusion of mimics, while management uses anti-inflammatory and immunomodulatory therapy alongside measures to preserve joint function. The peer-reviewed research associated with this topic addresses juvenile idiopathic arthritis in different populations, knee and septic arthritis, cytokine and genetic determinants of inflammatory and autoimmune disease, and anti-inflammatory interventions, reflecting the field's focus on the immune mechanisms underlying joint inflammation, the systemic and autoimmune context in which it arises, and the strategies used to control inflammation and limit articular damage in chronic inflammatory joint disease.
